Practical support to help you access the community, attend appointments, and manage everyday tasks such as shopping and errands. Our support workers work alongside you to build your confidence and independence, not replace it.
Who Is This Service For?
Anyone who needs assistance navigating daily life outside the home, including GP visits, hospital appointments, and supermarket trips. Particularly beneficial for people who have recently been discharged from hospital or who are rebuilding their confidence after an illness or injury.
